New Clients Information
The Parent Intake serves as a preliminary evaluation and in many cases, no further evaluation is required. During the Parent Intake the goals and the concerns of the parents are identified and a placement is made according to the decisions made by the clinic director.
- Step 1: Interest in services at the Lighthouse Project is expressed through a phone call: 408.871.8711, or email to: .
- Step 2: An appointment is made and a $50 deposit is required to hold your parent intake appointment.
- Step 3: After the appointment is made, the office can mail you the Registration and Sensory Profile Evaluation forms to fill out by hand. Or you can download all three forms below, and email them back to once completed.
- Step 4: During the Parent Intake, the Clinic Director and parents will discuss their specific concerns and a weekly appointment time will be made to start interventions.
