New Client Intake Process

In order to efficiently and accurately provide occupational therapy services for you and your child, a number of steps must be taken. Here is a step- by- step description of the process that takes place:


1: Preliminary Information Request
Interest in services at The Lighthouse Project is expressed through a phone call or email.

2: Parent Intake packet mailed An information packet, including contact information, the child's history and special needs, billing lists, and other information is sent by The Lighthouse Project and mailed back by the recipient.

3: Parent Intake packet reviewed and appointment made. The clinic director reviews the information in the Parent Intake packet and an appointment time is set up.

4: Parent Intake Appointment In this one to two hour session, the parent(s) meet with the clinic director to thoroughly discuss the child's needs. Documents from other health professionals are reviewed and therapeutic interventions are recommended.

5: Parents contacted by therapist A therapist will contact the parent(s) to schedule appointments for assessment testing and treatment.
